It adds: “People who find it difficult to get enough vitamin B12 in their diets, such as those following a vegan diet, may need vitamin B12 tablets for life.”

Busting B12 myths

According to Harvard Health, the internet is awash with articles lauding the use of vitamin B12 to prevent Alzheimer’s disease, heart disease, and other chronic conditions.

“Most are based on poor or faulty evidence,” warns the health body.

It adds: “For now, it’s best to get enough vitamin B12 to prevent a deficiency, and not look to it as a remedy for what ails you.”

Low Vitamin B12 and low iron levels are causes of anaemia. Anaemia is when you do not have enough healthy red blood cells to carry the oxygen you need around your body. When anaemia first develops, symptoms can be loss of appetite, constipation, headaches, irritability and difficulty concentrating. Another major indicator that your B12 levels are low include mouth pain and muscle twitches.

Mouth pain

Vitamin B12 can affect the mouth causing pain.

The pain can be a burning sensation, a mouth ulcer or a swollen tongue.

These symptoms occur because vitamin B12 deficiency causes a reduction in red blood cell production, which results in less oxygen reaching the tongue.

Muscle twitching

According to a study in Neurology International published in August 2014, different vitamin deficiencies, including vitamin B12 deficiency, can cause muscle twitching.

But it’s also important to note twitches are common and rarely a sign of anything serious.

They usually go away on their own, but if a twitch lasts more than two weeks, speak to your GP.

Twitches are most often linked to stress and anxiety, tiredness and exhaustion, drinking caffeine or alcohol, and some medicines.

In a study published in the US National Library of Medicine National Institutes of Health, the many faces of a vitamin B12 deficiency was analysed.

The study looked at different patients admitted to hospital for a variety of painful symptoms caused by a vitamin B12 deficiency

The study noted: “Patient D is a 33-year-old woman with a history of fatigue, inertia, indolence, paraesthesia in her hands and feet, difficulties concentrating, problems with remembering things, and word finding disturbances.

“The most prevalent symptoms of vitamin B12 deficiency are neurologic, such as paraesthesia in hands and feet, muscle cramps, dizziness, cognitive disturbances, ataxia, and erectile dysfunction, as well as fatigue, psychiatric symptoms like depression, and macrocytic anaemia.”

Even if a person gets enough vitamin B12 in their diet, some underlying health conditions can affect the absorption of vitamin B12 in the gut.

These conditions include:

Crohn’s disease

Celiac disease

Atrophic gastritis

Pernicious anaemia

Treatment

To prevent vitamin B12 deficiency, it’s strongly advised for vegans they should take adequate amounts of vitamin B12 supplements to make up for the shortage in their diet.

For people who cannot absorb B12, the condition cannot be prevented.

However, once it is diagnosed, regular injections of vitamin B12 will prevent symptoms from returning.

Megaloblastic anaemia is “a hallmark of vitamin B12 deficiency”, added the National Institutes of Health (NIH).

This presents its own symptoms too, said the NHS, such as:

  • Extreme tiredness (fatigue)
  • Lack of energy (lethargy)
  • Breathlessness
  • Feeling faint
  • Headaches
  • Pale skin
  • Noticeable heartbeats (palpitations)
  • Hearing sounds coming from inside the body, rather than from an outside source (tinnitus)
  • Loss of appetite and weight loss

A vitamin B12 deficiency can be diagnosed following a targeted blood test.

This is usually in combination with a discussion of symptoms, as there is a limitation to only relying on the blood test.

The NHS explained: “A particular drawback of testing vitamin B12 levels is that the current widely-used blood test only measures the total amount of vitamin B12 in your blood.

“This means it measures forms of vitamin B12 that are ‘active’ and can be used by your body, as well as the ‘inactive’ forms, which cannot.

“If a significant amount of the vitamin B12 in your blood is inactive, a blood test may show that you have normal B12 levels, even though your body cannot use much of it.”

Should signs of a vitamin B12 deficiency not be seen to, complications can develop.

Neurological damage can occur, which may lead to difficulty walking or speaking.

It can also lead to a loss of physical coordination, which can affect your whole body.

If neurological problems do develop, they may be irreversible, highlighting the importance of speaking to a GP.

Treatment typically involves vitamin B12 injections, which will either be:

  • Hydroxocobalamin
  • Cyanocobalamin

“Hydroxocobalamin is usually the recommended option as it stays in the body for longer,” said the NHS.

Injections are typically administered by a GP or nurse every other day, for two weeks, until symptoms improve.

After this initial period, you might need to have an injection every two to three months “for the rest of your life”.

If the deficiency was left long enough in the first place to cause numbness and tingling in the hands and feet, you’ll be referred to a haematologist.

To ensure treatments are effective, you might need to have further blood tests.

A study published in MD Edge Neurology found that vitamin B12 deficiency may cause isolated facial neuralgia with sensations of a decrease in touch and pain, as well as numbness on the affected side.

“It was somewhat unexpected that vitamin B12 deficiency can cause isolated facial neuralgia,” said Dr Jitendra Baruah, a neurologist.

He continued: “Treatment for facial neuralgia is sometimes very difficult, and patients may often go into multimodalities treatment without much success.”

Those lacking B12 may also notice the colour of their face is more yellow. 

The whites of the eyes may also appear less white and more yellow.

The reason for this colour change in the face is due to the body’s red blood cell production being affected.

When the body lacks B12, it is not able to make red blood cells as easy.

People with a B12 deficiency often look pale or have a slight yellow tinge to the skin and whites of the eyes, a condition known as jaundice.

This happens when a lack of B12 causes problems with your body’s red blood cell production.

Due to the low levels of healthy red blood cells, leading to a type of anaemia called pernicious anaemia.

Symptoms of pernicious anaemia may include diarrhoea, loss of appetite, and jaundice.

How should I respond to these symptoms?

You should see a GP if you’re experiencing symptoms of vitamin B12 deficiency, the NHS says.

“These conditions can often be diagnosed based on your symptoms and the results of a blood test,” explains the health body.

It’s doubly important for vitamin B12 deficiency to be diagnosed and treated as soon as possible.

“Although many of the symptoms improve with treatment, some problems caused by the condition can be irreversible if left untreated,” warns the NHS.

Vitamin B12 deficiency: White spots on your forearms is a lesser-known sign of low levels

Vitamin B12 is an essential nutrient for good health. Most people can obtain it from a varied diet that includes animal products. However, people eating a plant-based diet can only get vitamin B12 from fortified foods or supplements.

One lesser-known symptom to be aware of is white spots on a person’s forearm.

For most people, a B12 deficiency should be easy to prevent simply by ensuring you are getting enough B12 in your diet.

Treatment of vitamin B12 deficiency

If a person isn’t getting enough vitamin B12 from their diet they may be advised by a GP to eat more foods fortified with vitamin B12 or to take regular supplements.

Vitamin B12 injections may also be recommended, and for those with pernicious anaemia, injections may be required for the rest of their lives.

Experts say adults aged 19 to 64 require around 1.5 micrograms (mg) a day of vitamin B12, and unless you have pernicious anaemia, you should be able to get this through your diet.

Vitamin B12 supplements are available in the form of tablets or liquids that a person can take under the tongue (sublingually).

People eating a plant-based diet may need additional vitamin B12 in the form of supplements, especially when pregnant or breastfeeding.

Additionally, older adults, people with gastrointestinal conditions, and those on certain medication, including proton pump inhibitors and metformin, may need to take a supplement.
