War fears EXPLODE as China invasion of Taiwan may ‘threaten survival of Japan’

Taro Aso, Japan’s deputy prime minister, has said that the US and Japan would have to band together to shield Taiwan in an event of conflict, according to Kyodo News. Japan’s leading news agency reported that Taro Aso said a conflict over Taiwan could be devastating for Japan.

“If a major problem took place in Taiwan, it would not be too much to say that it could relate to a survival-threatening situation (for Japan),” he said.

“We are closely monitoring the situation.”

Tensions have been rising between China and Taiwan as Beijing has suggested the possibility of invading their island neighbour.

On Tuesday, Zhao Lijan, a spokesperson from China’s foreign ministry, warned “no one should underestimate the Chinese people’s staunch resolve, firm will, and formidable ability to defend national sovereignty.”

Last week, Nobuo Kishi, Japan’s Defence Minister, announced that the security of Taiwan is directly linked to Japan and stressed the importance of a Japanese intervention.

“The peace and stability of Taiwan are directly connected to Japan and we are closely monitoring ties between China and Taiwan, as well as Chinese military activity,” he told Bloomberg.

“As China strengthens its military, its balance with Taiwan is tipping heavily to the Chinese side.”

This follows reports the US and Japan have been uniting forces and preparing for a conflict with China, conducting joint military exercises and war games.

Serious preparations for a conflict with China began in the last year of Donald Trump’s administration.

The reunification of Taiwan has been in the works for a long time and Kevin Rudd, former Australian prime minister, is concerned about the global implications.

“I think what we’ll then be moving into is a period which China will be looking at its options to leverage Taiwan back into a form of a political union with China by the time we get to the late 2020s and into the 2030s,” Rudd told CNBC.

“And that’s when I believe it does get dangerous for us all.”

Japan’s deputy prime minister also shared his concerns that China could shift its interests to other islands as well.

“We need to think hard that Okinawa could be the next,” he said, talking about a Japanese prefecture where China claims a number of small uninhabited islands including the Senkaku island.

OMT 'Best Insurance' for Extended Survival After Revascularization

When it comes to medical therapy after a coronary revascularization procedure, more is better. Patients started and then maintained indefinitely on more rather than fewer of the drugs identified as optimal medical therapy (OMT) achieve a major survival benefit 10 years later, according to long-term follow-up from an extended analysis of the SYNTAX trial.

For the survival benefit at 10 years, “the present study suggests that at least three types of optimal medical therapy should be maintained for at least 5 years after revascularization,” reported a multinational team of cardiovascular specialists led by Hideyuki Kawashima, MD and Patrick Serruys, MD, who both have affiliations with the department of cardiology of the National University of Ireland, Galway.

The SYNTAX trial was conducted to compare percutaneous coronary intervention (PCI) with coronary artery bypass grafting (CABG) for patients with previously untreated three-vessel and/or left main disease (N Engl J Med 2009;360:961-72). The conclusion from that study, published in 2009 and subsequently reinforced by a 5-year follow-up, was that CABG should remain the standard of care for complex lesions.

Optimal Medical Therapy Defined

In the course of SYNTAX, the impact of OMT on outcome was also evaluated in a subanalysis. At 5 years, there was a mortality advantage for those receiving an antiplatelet drug, a statin, a renin-angiotensin system inhibitor (ACE inhibitor or angiotensin receptor blocker), and a beta-blocker when compared with fewer of these agents.

When an investigator-initiated extension of SYNTAX, called SYNTAXES, was conducted to compare the outcomes of PCI and CABG at 10 years, it also permitted an extended analysis of OMT. Although the primary comparison of SYNTAXES, reported 2 years ago, did not show a significant difference between PCI and CABG for mortality at 10 years, there was a difference for OMT.

When investigators compared treatment with three or more OMT agents with that with two or fewer OMT drugs at 5 years, the result for all-cause death at 10 years translated into a more than 50% relative reduction (hazard ratio, 0.47; = .002). The absolute difference in mortality was a more than 6% reduction (13.1% vs. 19.9%).

OMT Data Offer Major Message

The current study is considered to have a major message for patients as well as physicians.

“OMT even outweighs the survival benefit from revascularization alone, so our patients should convince themselves of the value of rigorous adherence and compliance,” Serruys said in an interview. According to him, these are compelling data for telling patients that OMT “is the best insurance for extended survival.” We now know from these data “the longer, the better.”

The same message from these data extends to physicians.

“I wish I could understand the apparent blind spot physicians have with respect to prescribing OMT despite the overwhelming benefit from multiple clinical trials,” said William E. Boden, MD, professor of medicine, Boston University.

Boden was a coauthor of an editorial accompanying the newly published SYNTAXES subanalysis. In the editorial, he noted that OMT following revascularization and in other high-risk patients “has been unacceptably low,” but he was asked to expand on the lessons from the newly released SYNTAXES subanalysis in an interview.

“There has often been a belief that revascularization negates the need for OMT and that’s why the SYNTAXES trial 10-year mortality reduction — which builds upon an earlier 5-year mortality reduction analysis — is so important,” he said.

Patients Should Take OMT Long Term

These data “should be both a motivator for physicians to prescribe OMT and for patients to remain adherent to OMT,” he said. “It is the best warranty to blunt the progression of atherosclerosis and to reduce subsequent cardiac events.”

For the 10-year subanalysis of OMT in SYNTAXES, the patients were stratified by the number of OMTs they were taking at 5 years after revascularization and then evaluated for survival at 10 years. Of the 1,472 patients available for analysis at 5 years, only 678 (46%) were on OMT. The other 794 patients were not.

Graphically, the Kaplan-Meier survival curve for those on three types of OMT was consistently beneath that of those on four OMTs, but the gap narrowed over time. At the end of 10 years, the advantage of the four-drug OMT was not statistically significant relative to three or fewer (13.1% vs. 12.7%).

Statins and Antiplatelets Show Largest Effect

When analyzed individually and in different combinations, the agents with OMT did not appear to be equal. For example, the biggest survival gap at 10 years was for those who were on an antiplatelet therapy and a statin at 5 years relative to those who were not on either (13.2% vs. 22.6%; = .006). Even after adjustment, there was nearly 45% survival benefit for these two agents (HR, 0.556; P = .02).

Conversely, the 10-year survival advantage for being on a renin-angiotensin system inhibitor at 5 years versus not being exposed to this therapy was small and nonsignificant (14.7% vs. 13.7%; = .651).

The precise proportion of patients who were prescribed and adhered to OMT between 5 years and 10 years is unknown, acknowledged the authors, so conclusions are limited about the added benefit of 10- versus 5-year OMT, although the authors presume that a substantial proportion of those adherent for 5 years would likely continue on these therapies.

It can be said with confidence that those adherent for at least 5 years are more likely to be alive at 10 years than those who are not, according to Boden. He considers these data a call for physicians and all high-risk patients, not just those who have undergone revascularization, to take these standard therapies.

There are plenty of data to “show how poorly we treat patients with OMT,” said Boden, citing several studies. In one, which looked at OMT in a nationally representative sample in the United States, only a third of patients with angina were taking an antiplatelet, a statin, and a beta-blocker, all of which are indicated.

“Hospitalization for revascularization provides an opportune time to capture the attention of patients and their physicians,” he wrote in his editorial. He called OMT “an imperative to optimize clinical outcomes.”

Many of the investigators involved in the SYNTAXES subanalysis, including Serruys, have financial relationships with multiple pharmaceutical companies, including Boston Scientific, which provided the initial funding for the SYNTAX trial. Boden reports no potential conflicts of interest.

Prepare For Survival Platforming With Residual, Launching Later This Year

Games are often about one simple rule: Don’t Die. But none more so than survival games, which are more like: Don’t Die, But Also We Really Want You To Die. We’ve seen a fair few survival games in the last few years, many of which are bloody brilliant, and we’re now at that stage where we’re seeing survival-meets-other-genre games that mix things up a little.

With that in mind, Residual is a procedurally generated pixel art survival-platformer set in space. With the help of your handy-yet-annoying robot companion, you’ll be trying to stay alive in a hostile alien world as you try to find a way home. Spoilers: a lot of things want to kill you. It is a survival game, after all.

Residual will be coming to the Nintendo Switch in Q3 2021, so some time towards the end of the summer.

Lovecraftian Survival Horror Game Song of Horror is Available Now

From the days of Resident Evil and Silent Hill right through to the present, survival horror has long had a sizeable fanbase on console, which is why we’re so excited to bring Song of Horror to Xbox One and Xbox Series X|S. A lot of the series that inspired this Lovecraftian, third person adventure launched on Xbox consoles of old, and part of Song of Horror’s DNA is to pay homage to these classic masterpieces of the survival-horror genre.

As studio co-founder Carlos G.F. Grupeli says, that’s exactly why bringing Song of Horror to console felt like the next logical step after making a splash on PC in 2019. “The survival-horror genre has such a long and  rich history on Xbox that it feels like absolutely the right home for our little ode to all that has gone before,” says Carlos. “We want to bring it to an audience that truly appreciates real scares.”

Song of Horror charges you with investigating the mysterious disappearance both of the famed author Sebastian P. Husher, as well as his entire family. Just to confound the mystery further, Husher’s editor sent an assistant to his house to look for him, but he too vanished without trace. These disappearances spark a set of events lifts the lid on something truly dreadful: a nameless, dark entity known only as The Presence, who seems to be the dark force behind this mystery.

Outbreak: Endless Nightmares Brings Co-Op Survival Horror To Switch Next Week

Launching next week on Nintendo Switch is Outbreak: Endless Nightmares, a new co-op survival horror from Dead Drop Studios.

Set right after the events of Outbreak: The Nightmare Chronicles, Endless Nightmares kicks things off with players trapped inside the remains of Arzt Memorial Hospital – a hub where you can organise your inventory and storage cache, and level up your character. Just like before, you’ll head out from this safe spot to explore, gather supplies, find clues, and fight your way through hordes of the undead in a “true horror” roguelite experience.

“Outbreak: Endless Nightmares delivers extreme replayability by offering multiple character options – each with their own abilities, upgrade paths, and more. Explore, scavenge for supplies, uncover clues, and fight your way through the anomalies – each consisting of semi-procedurally generated instances where both the environment and the undead are out to kill you!

Better Survival After Lumpectomy Plus RT vs Mastectomy

Overall survival and breast cancer–specific survival were consistently better for women with early breast cancer who were treated with breast conservation surgery (BCS) followed by radiotherapy (RT) compared to women who were treated with mastectomy (Mx) with or without RT, a large Swedish cohort study indicates.

“Women should be strongly advised to not get more extensive surgery than necessary to excise the tumor ― we need to excise the tumor with clear margins, but [patients] do not benefit from further removal of noncancerous tissues,” commented lead author Jana de Boniface, PhD, Capio St. Göran’s Hospital, Stockholm, Sweden.

“Women may experience a feeling of safety from having the whole breast removed, but they should be counseled that a mastectomy does not result in a better prognosis — probably rather the opposite,” she told Medscape Medical News in an email.

There are also “benefits for quality of life, body image, and social functioning that are reported by many women if they keep as much of their own body intact as possible,” she added.

The study was published online on May 5 in JAMA Surgery

“The most important message from this study is that BCS is safe for our cancer patients, and this confirms data from many prospective randomized trials that it yields outcomes that are at least as good as mastectomy,” commented Lisa Newman, MD, MPH, Weill Cornell Medicine, New York City, who authored an accompanying commentary.

Breast Cancer Register

For this study, de Boniface and colleagues collected data prospectively from the Swedish National Breast Cancer Register on a total of 48,986 patients.

“We included all patients diagnosed as having primary invasive breast cancer from January 1, 2008, until December 31, 2017, who underwent breast surgery,” the investigators explain.

The team took into account comorbidities and the patients’ education, income, and country of birth, as identified from several databases across the study interval.

Women underwent surgery as follows:

  • 59.9% underwent locoregional treatment with BCS with RT (BCS+RT);
  • 25.3% underwent a mastectomy followed by RT (Mx+RT);
  • 4.7% underwent mastectomy without RT (Mx-RT).

The median follow-up was 6.28 years.

Slightly more than one third (35.2%) of deaths that occurred during follow-up were due to breast cancer, the authors note. At 5 years, 91.1% of patients were still alive; the breast cancer–specific survival rate was 96.3%.

However, the breast cancer–specific survival rate was 66% worse among the women who had MX+RT compared to those who had BCS+RT (hazard ratio [HR], 1.66).

The overall survival was 79% worse following MX-RT compared to BCS+RT (HR, 1.79), after adjustment for tumor characteristics, treatment, demographics, comorbidity, and socioeconomic background. (It is well established that mastectomy is more commonly performed in women of lower socioeconomic status as well as in women with comorbidities, de Boniface commented.)

“Honestly, we would have expected that comorbidity and socioeconomic status, being factors that affect treatment choice, would have abrogated any survival differences between the surgical groups, but seeing that this is actually not the case is puzzling and highly interesting, although we have not found any clear causal relationship yet,” de Boniface noted.

She also noted that most women have the option of undergoing BCS, even women with large tumors; it has been shown that they, too, can be safely treated with BCS.

“It is striking that extensive breast surgery is more prevalent in node-positive disease despite suitability for breast conservation, indicating a misconception of safety, probably both from a patient and a physician perspective,” the authors observe.

“This report casts additional doubt on the practice to offer mastectomy to patients who are suitable candidates for breast conservation,” they conclude.

Role of RT

In her commentary, Newman writes that these “fascinating results…challenge the wisdom of prematurely abandoning radiation after BCS for clinically early-stage disease.”

Commenting further to Medscape Medical News, she pointed out that postmastectomy RT was given routinely to the highest-risk mastectomy patients (namely, patients with node-positive disease), so outcomes among these high-risk patients cannot be compared on the basis of whether or not RT was given.

“Similarly, radiation was typically not given to the mastectomy patients with favorable biology — only 3.5% of T1N0 mastectomy patients received RT — so we cannot compare outcomes in these early-stage mastectomy patients based upon whether or not radiation was delivered,” she added.

Moreover, patients who underwent BCS were excluded from the analysis if they did not receive RT, suggesting that physicians may speculate that RT contributed to the survival advantage seen in the BCS group.

However, this does not mean that RT definitively explained the survival advantage in this cohort, she said.

“Ongoing studies evaluating omission of RT as another strategy to deescalate breast cancer treatment will need to be evaluated in the context of these data [that] suggest a possible survival benefit from radiation,” Newman acknowledged.

  • Explore the vastness of space: The vast emptiness of space is breathtaking, really. There is plenty of areas to traverse in Breathedge, with the zero-gravity environment offering unique angles to explore asteroids and wreckages. During your journey, you’ll come across strange yet cinematic locales, such as freezing asteroids or ruined mayonnaise freighters. Each area offers lots of quests and objectives, fueling exploration and making each adventure out into space a unique and rewarding experience.
